How do I know I have this issue?

Your crank arm will be coming loose, typically on the non-drive side. It will typically get wobbly, and then it can also fall off in extreme cases.

What causes this issue?

Over time, the crank arm may come loose. In some cases, this can also be caused by incorrect assembly.
